You have the right to receive a copy of this notice, to inspect and obtain a copy of your record, to request corrections, to request confidential communications, to ask for limits on what is used or shared, to receive a list of certain disclosures, and to be notified of a breach affecting your information.

You may file a complaint if you believe your rights have been violated, and you will not be retaliated against for doing so.

Health information may be used and disclosed to provide and coordinate your care, to obtain payment for services, and to run the practice — for example, scheduling, record-keeping, and quality review.

Most uses and disclosures of psychotherapy notes, uses for marketing, and any sale of information require your written authorization. You may revoke an authorization in writing at any time.

Certain disclosures may be permitted or required without your authorization — for example, to prevent a serious and imminent threat to health or safety, for required public-health activities, in response to a valid court order, or where a mandatory reporting duty applies under Nevada law.

Sessions are held over secure video. Scheduling, intake documents, clinical records, messaging and payment are handled through the practice's client portal.

Ordinary email and website forms are not secure and are not used to collect detailed health information. The inquiry form on this website asks only for what is needed to arrange a first conversation.

The practice is required by law to maintain the privacy and security of protected health information, to notify you promptly if a breach occurs that may have compromised it, and to follow the duties and privacy practices described in the current notice.

If this notice changes, the revised version will be published on this page with a new effective date.
